Question 2. How do I get these services?
There are some services that you can access through the Commonwealth Home Support Program (CHSP). These include:
· social support-individual (assistance with shopping and bill paying, outings, friendly visiting)
· social support groups
· domestic assistance and spring cleaning
· home maintenance (such as changing light bulbs or smoke alarm batteries)
· gardening (such as pruning, weeding, yard tidying up)
· transport to medical and other appointments or transport to social support groups
You should be registered with My Aged Care before you can receive these services. You can contact My Aged Care on 1800 200 422 or visit their website https://www.myagedcare.gov.au and use the Make a referral tool
If you have more complex needs, a Home Care Package (HCP) may be right for you. You can access similar services through CHSP, but on top of this, the HCP services are coordinated and tailored to meet your specific needs. HCP services include but are not limited to: personal care, nursing assistance, in-home respite, home modifications, meal preparation, domestic assistance, gardening, transport, home maintenance, allied health professionals, social support.
Some facts about Home Care Packages:
· there are four levels of packages to cater for different needs: Level 1 (Basic care needs), Level 2 (Low care needs), Level 3 (Intermediate care needs), Level 4 (High care needs
· a formal assessment by a member of an Aged Care Assessment Team (ACAT) is required before you can get a package
